NURS-643 Primary Care of the Adult-3 (3 semester credits / 84 clinical hours)

 

How the changes support the School of Nursing Graduate Program Outcomes

 

Students who complete the Master of Science in nursing program will have developed the ability to perform Nurse Practitioner skills in a variety of healthcare settings.  The proposed program changes will enhance the student’s abilities in the following program outcomes by providing:

 

 

 

 

§         Increased class time for the evaluation of theories and concepts drawn from nursing, the arts and humanities, and the biological, social, and physical sciences as the basis for practice as a nurse practitioner.

 

§         Exposure to a greater variety of clinical settings in order to demonstrate competence as a nurse practitioner in a medical practice environment.

 

§         More opportunities to collaborate in interdisciplinary efforts to provide health care and improve healthcare delivery systems to local, state, and/or national levels.

 

§         Additional time to be involved in the development of proposals for studies or projects that contribute to the expansion of nursing knowledge and practice.

 

§         Increased clinical mentoring opportunities to allow for the development and demonstration of ethical decision making and professional accountability as a nurse practitioner.

Describe the rationale for the proposed program change(s):

 

#1 Problem Identified:

 

§         The workload in NURS-617 (Preceptorship-1) and NURS-627 (Preceptorship-2) are not in congruence with other 3 credit clinical courses in the Nurse Practitioner Program. The standard workload limit for a clinical course is 3 class hours and 6 clinical hours per week, which equals 9 contact hours per week. This calculates to 42 classroom hours and 84 clinical hours per semester.  

 

§         The preceptorship courses require 126 clinical hours per semester, which is on average 9 clinical hours per week or 9 contact hours per week.  Requiring students to complete 9 clinical hours per week does not allow for classroom hours to be incorporated into the courses without exceeding the 9 contact hour a week limit.

 

§         Classroom hours however, have been incorporated into the courses in order to cover required program material to adequately prepare students as Nurse Practitioners and to meet the standards mandated by accrediting boards.     

 

Solution:

 

§         Replace NURS-617 and NURS-627 with courses NURS-644 (Nurse Practitioner Preceptorship-1) and NURS-645 (Nurse Practitioner Preceptorship-2). Reduce the clinical hours in both NURS-644 and 645 to 84 clinical hours, from 126 clinical hours. This is a decrease of 42 clinical hours from each course per semester. The reconfigured course will then be in congruence with the standard workload limit for a clinical course of 3 classroom hours and 6 clinical hours per week.    

 

 

 

 

Rational:

 

§         The change will bring the two courses into alignment with the approved clinical course workload of 9 contact hours per week (3 class hours and 6 clinical hours) and in alignment with all other 3 credit clinical courses in the Nurse Practitioner Program (42 classroom hours and 84 clinical hours per semester.)

 

§         Currently, there are limited classroom hours due to the clinical hour requirement for these courses. The change allows for 84 hours of classroom time, not available currently due to workload limits, to cover required program material.

 

§         The new course numbers reflect the new program sequence of courses in an intuitive and logical manner.

 

 

 

#2 Problem Identified

 

§         The above change decreases the total clinical hours in the Nurse Practitioner program by 84 hours. Research indicates that program outcomes decrease when clinical hours are reduced in Nurse Practitioner programs.

 

Solution:

 

§         Add a 3 credit clinical course to the program, replacing the 84 clinical hours.  This also provides 42 more classroom hours.  Renumber and rename two existing clinical courses

 

NURS-625 (Episodic care of the Adult) change to NURS-641 Primary Care of the Adult-1

NURS-626 (Chronic Care of the Adult) change to NURS-642 Primary Care of the Adult-2

Rational:

 

§         The current Nurse Practitioner program faculty struggles to cover all the material that Nurse Practitioner students are required to know to function safely and effectively in a clinical environment upon graduation. The current program structure does not provide a mechanism for the incorporation of additional course materials or requirements from accrediting or professional agencies.

 

§         The Consensus Model for APRN Regulation: Licensure, Accreditation, Certification and Education, released July 2008, revised the Adult Nurse Practitioner area of focus to incorporate the care and treatment of the geriatric population to a greater degree.  This new model requires incorporation of additional program content into the Nurse Practitioner Program so that the new standards can be effectively achieved.

 

§         The addition of a 3 credit course will allow for expanded coverage of currently required topics (e.g., hypertension, diabetes, heart disease) that have not received the amount of time required for in-depth student understanding. The additional course will allow time to enhance and infuse expanded coverage of geriatric focused material into the program.

 

 

 

 

 

 

 

Summary:

 

§       The above changes add an additional 3 credit course to the Family Nurse Practitioner and Adult Nurse Practitioner concentrations.

 

§       The Family Nurse Practitioner concentration will increase from 43 to 46 required credits

 

§       The Adult Nurse Practitioner concentration will increase from 40 to 43 required credits

 

§       The total number of clinical hours for both concentrations remain the same

 

§       There are four course number changes and two course title changes
